Nursing Team

The nursing team at the Cedar Brook Practice has a wide range of specialist expertise in Asthma, Diabetes, Respiratory Disease, Hypertension, Coronary Heart Disease, Sexual Health, Child Health, Family Planning, Hormone Replacement Therapy, Well Man, Well Woman, Diet and Life Style Advice and Travel Health and Immunisations.

Chronic Disease Management

Regular monitoring for patients with Chronic Disease conditions are undertaken by the GPs and Nurses. If you are unsure whether to see a Nurse or Doctor please ask at reception and we can advise you.

Children’s Immunisation Clinics

Our children immunisation clinics are run on various days of the week by one of our Practice Nurses.  Please book an appointment at reception. Your Health Visitor will be able to give full details of the immunisation programme for under 5’s and older children.

Interpreting Service

If you require an interpreter for your appointment with the doctor, nurse or pharmacist please advise the receptionist when booking your appointment. This service is provided by telephone, must be booked in advance and is subject to availability.

Influenza Vaccination

In September each year we offer free flu vaccinations to high-risk patients.

Please book a flu vaccine appointment if you are over 65 or you are a carer or have a history of heart disease, diabetes, respiratory disease, liver or renal problems or live/work in institutions or nursing homes.

Contraceptive Services

The Practice offers a full range of contraceptive advice and treatments, including contraceptive injections, Mirena and Contraceptive Implants. Insertion, annual check up and removal of Intrauterine Contraceptive Devices (IUDs—Coils).

Thinking of starting a family?

If you are considering becoming pregnant, come and talk to our Nurses. We can provide you with a full range of pre-pregnancy advice including advice on folic acid, diet, exercise and abstaining from alcohol and cigarettes.

Coronary Heart Disease (CHD)

Annual monitoring for all patients with Coronary Heart Disease (CHD, angina, heart failure, history of heart attack or stroke) is offered at the surgery by the nurses.

Hypertension (High Blood Pressure)

Regular monitoring and annual reviews for patients with hypertension (raised blood pressure) are undertaken by the GPs and Nurses.

Respiratory Disease (Asthma, COPD) Clinic

These clinics are run by our practice nurses, offering regular monitoring and Annual Reviews. Spirometry testing is also available.

Sexual Health Services

A comprehensive family planning service is available during normal surgery hours led by the Practice Nurses.

Cervical smears are recommended for all women aged 25-50 years every three years, and every five years for women aged 50-65 years of age. These are carried out by appointment during surgery hours by one of the Nurses.

Sexually Transmitted Infection (STI) screening (including Chlamydia) is available from the Nurses and Doctors.

Menopause advice and Hormone Replacement Therapy advice is available from the Nurses and Doctors.

Cryotherapy

If you would like to access this service please consult your Doctor.

Warts and Verrucas

Dr Carter runs a monthly wart treatment clinic using Liquid Nitrogen to freeze-treat warts and Verrucas.

Diabetes Clinic

Dr Sira runs the Practice’s Diabetic Service and offers a wide range of services including annual reviews and follow-up appointments.

Diet Advice & Weight Management

The Practice Nurses offer dietary and lifestyle advice to patients, family and carers. They are able to advise and support patients wishing to lose weight.

Free NHS Health Check (for those aged 40 to 74)

These clinics are run by our practice nurse and health care assistant and the appointment will last approximately 30 minutes.  Please enquire at reception.

Maternity Self Referral

Did you know that if you are pregnant and would like to go to Hillingdon Hospital or Queen Charlotte and Chelsea Hospital to have your baby, you do not need a self referral from your GP.  You can now contact a midwife as soon as you find out you are pregnant to speed up your access to maternity care.

The hospital will pass on the information to your GP.

Tests & Results

Samples

These should be handed into reception before 11:00. These are collected before Midday by the hospital courier. Please ensure any specimen bottles are securely capped / closed, attached to a fully completed request form and are labelled with your name and date of birth, or the hospital may be unable to carry out the test. Sample containers and advice on their use are available from reception. Tests to check for infections (swabs, urine, faeces, sputum etc) take a minimum of 5 working days to be reported by the laboratory as they need time to grow bugs if they are present.

Blood Tests, X-Ray and Ultrasound Investigations

Blood tests are normally carried out at Hillingdon Hospital Phlebotomy Department open from 08:30 to 16:30 on weekdays (no appointment necessary), or by appointment at The Hesa Centre, 52 Station Road, Hayes. You will need to book an appointment at the Hesa Centre on 01895 484800. Routine blood tests are reported from the laboratory to the Practice usually within 5 working days. More complex or specialised tests may have to be sent away to specialist laboratories and may take weeks to be reported.  Blood tests for elderly & disabled patients can be carried out by our health care assistant at the practice.  Please book an appointment at reception.

Results of Investigations

Please phone the surgery after 11:00 seven days after your tests. To safeguard your confidentiality you will be asked a number of questions to confirm your identity. We strive to maintain high standards of confidentiality for our patients therefore you may occasionally be requested to attend the surgery in person to discuss your results, providing confirmation of your identity (e.g. passport, driving licence, etc.) when you attend.

Cervical Smear Results

These will be sent by letter from the Health Authority. If you have not received your results 6 weeks after your smear please contact the Practice.

Blood Tests

A blood test is when a sample of blood is taken for testing in a laboratory. Blood tests have a wide range of uses and are one of the most common types of medical test. For example, a blood test can be used to:

• assess your general state of health
• confirm the presence of a bacterial or viral infection
• see how well certain organs, such as the liver and kidneys, are functioning

A blood test usually involves the phlebotomist taking a blood sample from a blood vessel in your arm. and the usual place for a sample is the inside of the elbow or wrist, where the veins are relatively close to the surface. Blood samples from children are most commonly taken from the back of the hand. The child's hand will be anaesthetised (numbed) with a special cream before the sample is taken.

X-Ray

An X-ray is a widely used diagnostic test to examine the inside of the body. X-rays are a very effective way of detecting problems with bones, such as fractures. They can also often identify problems with soft tissue, such as pneumonia or breast cancer.

If you have a X-ray, you will be asked to lie on a table or stand against a surface so that the part of your body being X-rayed is between the X-ray tube and the photographic plate.

An X-ray is usually carried out by a radiographer, a healthcare professional who specialises in using imaging technology, such as X-rays and ultrasound scanners.

Certificates

The Practice is happy to complete the following private certificates and forms for a fee. A list of these current fees can be obtained from reception.

Payment can be made by cash or cheque only (Payable to The Cedar Brook Practice).  Please note that for some items a payment in advance will be required.

Holiday cancellation forms:

Can only be completed if you have consulted a Doctor for the illness causing the cancellation.

Mortgage or Insurance Forms:

Most of these forms can be completed by the Doctor without seeing the patient. Please allow a minimum of 4 weeks for completion.

Please note there is a fee for the completion of these forms.

Medical assessments and reports:

The Practice is happy to carry out specialist medical examinations or make reports as required for employment or licensing purposes. Examples include:

Medical examinations for Heavy Goods Vehicle (HGV), Taxi, Public Transport, Driving Licences, Fire Brigade Medicals and Adoption and Fostering Medicals/Reports. 

Please note there is a fee for these medicals. 50% will be payable in advance when the appointment is booked.

What the NHS App does

Connecting GP surgeries to the NHS App

Patients at 95% of GP surgeries in England can now use all the features of the NHS App.

You can check if your GP surgery is connected when you open the app for the first time. If it's not, you can register your email address, and we'll notify you when they go live.

If your GP surgery is not connected, you can still download the app and use it to check your symptoms and find out what to do when you need help urgently.

Keeping your data secure

When your surgery is connected, and you register in the app, we'll carry out checks to confirm your identity. The app will then securely connect to information from your GP surgery. To keep your access secure, we'll send a security code to your phone each time you use the app.
